The following example represents a standard Calibration sticker which can either be computer generated or purchased at most office supply companies. The completed sticker must be applied to the instrument indicating fitness for use. CALIB-2003 Calibration Sticker

Issue Date: 12/31/99 Approved By: __________________ Since the Calibration Sticker is so small and/or purchased, it is possible that the proper identification may not be on the sticker itself, as in the example above. An acceptable means of controlling such a "form" is to attach it to a sheet of paper indicating: the form number (CALIB-2003 in this case), the revision level, the issue date and the authorizing signature. This process can be used with most purchased forms so long as the process is defined within the Document Control procedures.
